Port Chester High School Senior Permission Slips
PCHS seniors who wish to leave campus during the school day when they have no scheduled periods or lunch should do the following:
- Complete the permission slip linked below.

- A parent must sign where indicated.
- Email completed permission slip to D. Paniccia. (Email address available in Staff Directory)
To avoid delay, please make sure your permission slip is legible and includes the student's name.
